REQUIREMENTS FOR THE SACRAMENTS

The following required documents and/or seminars are issued by the parishes in the Diocese of Malolos for the reception of some of the Sacraments.

SACRAMENT OF BAPTISM:

Infant Baptism:

  • Birth Certificate from either the National Statistics Office (NSO) or the Office of the Civil Registrar.
  • Seminar for Parents and Godparents (usually held either before the Baptism schedule or a few days before).

Adult Baptism:

  • Birth Certificate from either the National Statistics Office (NSO) or the Office of the Civil Registrar.
  • Seminar for the adult to be baptized (usually held either before the Baptism schedule or a few days before).
  • Profession of Faith (Recitation of the Apostles’ Creed) before the priest/deacon who will baptize.

SACRAMENT OF CONFIRMATION:

  • Birth Certificate from either the National Statistics Office (NSO) or the Office of the Civil Registrar.
  • Baptismal Certificate from the parish where the candidate was baptized.
  • Seminar for the young adult/adult to be confirmed (usually held either before the Confirmation schedule or a few days before).
  • For young candidates for confirmation (from schools), it shall be scheduled with the respective celebrant (Bishop/Delegated Priest) through the parish office. For candidates for confirmation in preparation for Marriage, it shall be scheduled with the Parish Priest.

SACRAMENT OF THE HOLY EUCHARIST (FIRST HOLY COMMUNION):

  • Birth Certificate from either the National Statistics Office (NSO) or the Office of the Civil Registrar.
  • Baptismal Certificate from the parish where the candidate was baptized.
  • Seminar for the young adult/adult to receive First Holy Communion (usually held either before the schedule or a few days before).
  • For both young candidates for First Holy Communion (from schools) and for candidates for First Holy Communion in preparation for Marriage or from the adult Catechumenate, it shall be scheduled in the parish office.

SACRAMENT OF HOLY MATRIMONY:

  • Baptismal Certificate (Latest, with annotation: “For Marriage Purposes”)
  • Confirmation certificate (with annotation: “For Marriage Purposes;” Confirmations to be scheduled in the parish office)
  • List of sponsors (maximum of 10 pairs, with address)
  • Marriage license (1 original and 3 photocopies)
  • Certificate of No Marriage from National Statistics Office (also called CENOMAR; 1 photocopy)
  • Marriage contract (for civil-married couples only)
  • Interview (please see parish office)
  • Family Life/Pre-Cana Seminar (to be scheduled in the Parish Office)
  • Confession (to be scheduled in the parish office or to be held before the wedding)
  • Wedding candles, candles for offering, bible, 2 pcs. rosary, crucifix, host and wine (please see parish office for details).

For the Sacraments of Reconciliation (Confession) and Anointing of the Sick, please contact the parish office for scheduling.

For the Sacrament of Holy Orders, the conferment is given to young male candidates who have undergone seminary formation, with the Canonical requirements of Philosophy and Theology, particularly at Immaculate Conception Major Seminary in Guiguinto, Bulacan for seminarians of the Diocese of Malolos.
